This role involves providing patient care within a home health setting, requiring flexibility in territory and adherence to agency protocols. The position offers a Monday through Friday schedule with on-call responsibilities, focusing on geriatric patient populations. Job Details: Location: Lorain, Ohio Schedule: Monday through Friday; 8:00 AM to 5:00 PM, 40 hours per week Required Qualifications: Certification in Basic Life Support (BLS), active Ohio nursing license in hand, minimum of 1 year of relevant experience including Oasis experience for staff completing Start of Care (SOC) in home health, proficiency in Homecare Homebase (HCHB), and a valid drivers license with personal transportation. Additional Qualifications: Experience with patient charting, timely communication and documentation, flexible with territory, and able to participate in on-call rotation. Responsibilities: Providing comprehensive home health nursing services, managing a patient load with 5-7 visits per day, ensuring compliance with clinical standards, and documenting care accurately. Orientation: Monday orientation required. On-Call & Weekend Coverage: On-call rotation is required; weekend on-call responsibilities are included. Travel & Transportation: Acceptance of travel pairs; each traveler must have own transportation. Mileage reimbursed at IRS standard rate. Additional Notes: All travelers must be fully vaccinated against COVID-19 and flu, have an active drivers license, and be flexible with territory. No first-time travelers accepted. Timely communication and documentation are mandatory.
