Job Description: Social Worker/Mental Health Therapist
Location: Family Medicine Associates (Grande Prairie)
Schedule: Monday - Friday 8:30-4:30 pm
FTE: 1.0
The Social worker/Mental Health Therapist at Family Medicine Associates is responsible for supporting physicians and helping patients of the clinic develop skills and resources they need to enhance their social functioning and social environments. Within the scope of practice, you may be asked to assess, and provide psychotherapeutic interventions. When needed, you will be responsible for the coordination of patient services, and then the referral of patients to external supports or health care providers.
Duties and Responsibilities
Provide clinical biopsychosocial assessments using a variety of assessment methods to assess clinical mental health problems.
Participate in early detection of emerging patient emotional, behavioral and mental disorders.
Form Completion (i.e.: Disability Tax Credits, AISH forms, Insurance Forms etc.)
Develop and implement clinical treatment plans
Advocate and for patients to meet patient and family needs such as food, housing, transport and financial support. This includes assisting patients with Income Support, AISH, emergency housing accommodations, emergency prescription, and suicide intervention.
Guides patients in the development of skills and strategies for handling their problems.
Provide short term individual and group counseling
Maintain a high level of current knowledge and trends in the relevant discipline specific skills and practices.
Support patients with diagnoses such as depression, anxiety, emotional regulation, stress management, self-esteem, life skills, career planning, grief and LGBTQ issues and any other presenting concerns.
Collect and maintain data for individual client and program evaluation.
Participate in multi-disciplinary case conferences and provide consultation and education to support other health care service providers.
Develop positive working relationships/partnerships with other disciplines, agencies and health care professionals in order to promote coordinated service, quality care and access to appropriate resources for clients
Qualifications and Skills
Registered Social Worker with ACSW, authorized to perform restricted psychosocial activities.
Criminal Record and Vulnerable Sector check
Bachelors degree in Social Work
Experience with providing therapeutic interventions and therapy to individuals or groups
Working knowledge of local and provincial mental health resources and community agencies/programs
Experience with capacity assessments is an asset.
Crisis counseling experience and the ability to deal effectively with patients with mental health concerns who are experiencing crises.
Understands and follows applicable legislation (e.g., Mental Health Act, Health Information Act, Privacy Act, other) And knowledge of mental health resources/programs.
Ability to communicate effectively orally, electronically or in writing
Ability to work in a busy medical office environment with minimal support
Good organizational and multitasking skills
Maintain absolute confidentiality of patient records
