Reporting to the Manager, the Social Worker works with the interdisciplinary team to develop a plan of care. The Social Worker conducts a bio-psycho-social assessment, applies social work theory and methods and develops care plans with the goal of maximizing social and emotional functioning, preventing problems and situations that reduce coping capacities and independence. The Social Worker addresses barriers to optimal recovery or adjustment and discharge from hospital as well as support in end-of-life situations.
Qualifications / Skills / Abilities
Successful completion of a Master of Social Work Degree.
Current registration in good standing with the College of Social Workers
Excellent professional judgment, conflict resolution and interpersonal communication skills with patients, families, team members, students and community partners
Competence in the practice of evidence-based counselling and supportive therapy and inter-professional care, with experience in the application of practice models and outcome measures
Experience with acute stroke patients, end of life and MAID situations
Working knowledge of Mental Health legislation and the Trillium Gift of Life process
Effective time management and organizational skills
Excellent written and verbal communication skills
Reliable, flexible, creative, self-directed professional able to work effectively and collaboratively with the interdisciplinary team as well as families and community partners
Two to five years of recent related experience in a medical clinical setting,
Able to work all shifts, 24/7, including weekends, and ability for multi-site coverage if required
Recent satisfactory performance and attendance records
