Summary of Duties To provide a full range of social work services to patients and their families admitted to the Trauma program. This includes psycho-social assessments, counselling to patients and families, discharge planning, crisis intervention, linkage to community resources, team consultation. Qualifications/Skills Masters of Social Work Degree Required Member of the Ontario College of Social Workers and Social Service Workers in Good Standing Minimum two (2) years of experience working in a Hospital setting Experience in Trauma preferred Exceptional communication skills required Knowledge of the HCCA and SDM act In-depth understanding of the Sunnybrook Policies that impact transitional planning Strong negotiation and conflict resolution skills Demonstrated skills in Interprofessional Collaborative Practice & Education Ability to balance collaborative practice with self-direction Ability to manage multiple tasks and set priorities in a fast-paced environment Commitment to student education Experience and commitment to quality improvement initiatives Demonstrated knowledge of organizational priorities and social work scope of practice priorities We thank all applicants for their interest.
