1.0 Full-Time FTE Status (37.5 Hours per week)
We are currently recruiting a Full-Time Social Worker (5 days, 37.5 hours per week, 1.0 FTE status) to join our team in Dufferin-Caledon, including Bolton.
Social Workers are a vital part of Dufferin Area Family Health Team (DAFHT) and collaborate with physicians and other interdisciplinary providers to achieve the vision and mission of the organization.
Our Social Workers are part of a dynamic, innovative, compassionate team dedicated to supporting, empowering, and collaborating with patients for improved health and well-being. This hybrid position will be in Dufferin-Caledon, including Bolton.
DUTIES AND RESPONSIBILITIES
Enthusiastic, effective and responsive in supporting, advocating for, and assisting with system navigation for individuals struggling with complex needs, including disability, poverty, illness, trauma, addiction, palliative, or mental health concerns
Knowledgeable of and able to assist individuals in navigating the application process for ODSP, Ontario Works, CPPD, Trillium, etc.
Skillful in working collaboratively with physicians, Nurse Practitioners, allied health providers, and community partners
Skillful in conducting psycho-social assessments and evaluating needs related to social determinants of health
Flexible, energetic, organized and committed to working from a trauma-informed, strength-based perspective
Respectful of diversity, including race, religion, culture, sexual orientation, and gender identity
Ability to travel within Dufferin and Caledon-Bolton, and comfortable conducting patient visits in office, in patient homes, and within the community
EDUCATION AND QUALIFICATIONS
MSW with 2+ years experience or BSW with 4+ years experience is required
Membership in good standing with OCSWSSW
Demonstrated case management, system navigation and referral coordination experience
Current up-to-date local knowledge of Dufferin-Caledon community and health services and PHIPPA
Proof of Criminal Records Check/Vulnerable Sector Screening
Valid Ontario Driver’s License, valid insurance, and access to a reliable vehicle
Working Conditions and Requirements
This is a hybrid, in-person, on-site role
Hours are 37.5 hours per week
Current driver’s license and reliable vehicle to travel to sites and within community
