Registered Nurse – Vascular Neurology/Stroke – UBMD Neurology Full-Time | Onsite Practice : UBMD Neurology Work Location : Buffalo, NY Pay Range : $39-$41 per hour Position Summary: The Registered Nurse – Vascular Neurology/Stroke will primarily support the Vascular Neurology/Stroke program through patient navigation, care coordination, and clinical triage. The position will also provide cross-coverage for clinical RN workflows across the neurology practice as capacity allows. Primary Responsibilities: Monitor stroke discharges from Buffalo General and ensure patients are scheduled for appropriate outpatient stroke follow-up. Provide ongoing stroke patient navigation and assist in preventing patients from being lost to follow-up. Coordinate post-visit care, including ensuring ordered testing, referrals, and other follow-up needs are appropriately processed and communicated. Triage patient phone calls, portal messages, and clinical tasks involving new or changing symptoms, medication questions, refill requests, and other clinical concerns. Escalate urgent or clinically significant concerns to the appropriate provider. Provide patient education and assist with care coordination as appropriate. Provide RN phone/task triage and clinical support for other neurology subspecialties when stroke workflow and staffing allow. Collaborate with providers, APPs, nursing, administrative staff, and outside facilities to facilitate continuity of care. As Vascular Neurology programs expand, potential opportunity to assist with office-based procedures and related patient care, including loop recorder implant procedures. Requirements: Current New York State RN license in good standing. Graduate of an accredited nursing program. Clinical nursing experience required. Neurology, stroke, ambulatory care, care coordination, or telephone triage experience preferred. Strong clinical assessment, communication, and organizational skills. Ability to independently prioritize clinical concerns and appropriately escalate urgent patient needs. Experience working within an electronic medical record and managing patient telephone/portal workflows preferred.
