JOB SUMMARY
Purpose of the role:
This position is responsible for providing day-to-day support to the multidisciplinary team by assisting with providing resident-centered care to the residents. The Registered Nurse is required to perform their duties in a manner that is consistent with the organization’s mission, vision, and values.
KEY ACCOUNTABILITIES & SUCCESS CRITERIA
Perform holistic resident assessments as required and assigned
Recognize, assess, and respond to resident health changes, including acute events
Daily monitoring of resident home areas, Registered Practical Nurses, and Personal Support Workers
Determine resident care requirements and development of resident plan of care including evaluation and revision of care plans
Consult with residents and or families for all changes, updates, and modifications to residents’ plan of care
Consult with physicians, community agencies, and institutions as required to meet resident needs
Advocate for resident and family choices and decisions
Administer medications and wound treatments as required
Document accurately and in a timely manner according to CNO standards
Delegate/teach and oversee skills and techniques to other staff/residents and/or families as required
Complete medication orders and quarterly reviews
Conduct and attend post-admission and annual care conferences
Assist with Quality Improvement by conducting audits as assigned
Report and complete medication error forms and initial follow up
Attend team meetings as required
Provide feedback on staff performance
Assist with writing and updating policies and procedures
The RN will work in accordance with all the policies and procedures of the facility.
Work in accordance with Ministry of Health and Long-Term Care regulations
Other duties as assigned
The employee will work safely and healthily and will follow the regulations outlined in the Occupational Health and Safety Act.
This position may be required to perform additional related job functions beyond this description.
QUALIFICATIONS & SKILLS
Graduate Nurse of a college or University program
Must be a current registrant in good standing with the College of Nurses of Ontario
Preferred completion of a geriatric assessment course
Minimum of two (2) years of experience in geriatric, rehabilitative, or long-term care (preferred)
Current CPR certificate – BCLS-C level required.
Membership with a professional association, an asset
Excellent team building and communication skills
Experience working and collaborating within a multidisciplinary team
Knowledge of Long-Term Care Standards and Occupational Health and Safety Acts
COMMUNICATION SKILLS
Ability to read and interpret documents as required.
Ability to communicate effectively with a multi-disciplinary team.
Excellent written and verbal communication skills
Effective and well-developed interpersonal/ customer service skills.
REASONING ABILITIES
Ability to solve practical problems and deal with a variety of concrete variables in situations.
Ability to interpret a variety of instructions furnished in written, oral, or diagram form using critical thinking skills.
