Key Responsibilities: Provide skilled nursing care to hospice and home health patients Conduct patient assessments, admissions, and routine visits Manage caseload and ensure continuity of care Develop and implement individualized care plans Coordinate with interdisciplinary teams Maintain accurate and timely documentation Meet productivity expectations of 24-30 visits per week Requirements: Active RN license (PA) Hospice experience required Home Health experience preferred Experience with HomeCare HomeBase (HCHB) preferred Candidates without HCHB may be considered at a reduced rate during initial training Strong clinical and communication skills Valid driver's license and reliable transportation Additional Details: Pay Rate: $58/hour + Mileage reimbursement provided (IRS rate) Coverage Area: Lower Bucks, Eastern Montgomery, Northeast Philadelphia (within ~60-mile radius) (Coverage may extend across service areas as needed) Overtime must be pre-approved with written authorization Why Join Us?
