Caseload Area
1to1 Rehab is currently recruiting a part-time Registered Dietitian (Independent Contractor) to support an adult community-based caseload in
Georgina
under secure Ministry funding.
Services are provided in clients' homes, long-term care homes, retirement residences, and other community settings. This role offers the opportunity to work with adults experiencing a wide range of nutritional and health needs while maintaining flexibility and control over your practice.

Responsibilities
Engage clients, families, caregivers, and care teams to establish meaningful client-centered goals
Complete comprehensive nutrition assessments and develop evidence-based treatment plans using measurable outcomes
Provide nutrition counselling, education, and intervention to support optimal health, safety, and quality of life
Monitor nutritional status and adjust care plans based on clinical needs and progress
Collaborate with physicians, rehabilitation professionals, caregivers, and community partners
Contribute to discharge planning and community referrals as appropriate
Maintain accurate and timely clinical documentation in accordance with professional standards, organizational policies, and funding requirements
Demonstrate professionalism, clinical reasoning, and sound judgment in community-based practice
Qualifications
Current registration in good standing with the College of Dietitians of Ontario
Compliance with the Regulated Health Professions Act and all applicable professional standards
Bachelor’s degree from an accredited Nutrition and Dietetics program
Experience working with adult populations
Community-based practice experience is preferred
Experience supporting clients in long-term care, retirement, or home care settings is considered an asset
Excellent assessment, intervention, education, and communication skills
Ability to work independently while collaborating effectively with an interdisciplinary team
Professional liability insurance
Valid driver's licence and access to reliable transportation
Current Vulnerable Sector Check or willingness to obtain one
