As a member of the community Seniors Mental Health Behavioral Support Services (SMH BSS) team at Providence Care, the Psychogeriatric Resource Consultant (PRC) plays an important role in capacity building and development across the continuum of care. The PRC supports team members across the system who care for older adults living with complex cognitive, mental health, addiction, and physical needs with associated responsive behaviours. The PRC supports staff/leadership in Long Term Care Homes, Acute Care, Ontario Health at Home and other related community agencies through capacity enhancement, consultation, and development activities. The PRC helps to facilitate an integrated system of care through the development and facilitation of knowledge/learning networks and participating on various working groups or system level networks with the goal of improving the experience for the person and their care partner.
Education & Experience
Degree in a Health Care Discipline; Masters’ level preferred
Registration with the Ontario professional College of discipline or eligibility to obtain
Minimum of three years of experience in community healthcare and/or Long-Term Care Home settings
Experience and expertise in seniors mental and behavioural health assessment, consultation, intervention strategies, and care planning for the defined population
Knowledge of and experience in working within the PIECES framework; experience as a PIECES educator preferred
Knowledge of and experience with Gentle Persuasive Approaches (GPA); Certified GPA Coach preferred
Experience in identifying staff learning and development needs, and developing educational interventions
Experience in providing education to a variety of sectors including the application of the principles of adult learning
Experience in community settings across a variety of parameters including direct care, consultative role, and program development
A combination of relevant education and experience may be considered
Position Duties & Responsibilities
Provides case-based consultations and education to healthcare providers along the continuum of care for the defined population, including Long Term Care, Primary Care, Acute Care, Ontario Health at Home, and other related community agencies
Develops effective working relationships with community healthcare partner organizations serving the defined population
Assists in identifying community learning and development needs, in collaboration with community partners
Participates as an integrated member of the SMH BSS team for the assigned geographic area
Provides system support for the implementation of effective service delivery models for the defined population to enhance person-centred care and improve outcomes in Long Term Care, Primary Care, Acute Care, Ontario Health at Home, and other related community agencies
