Clinical Care Manager
Primarily responsible for regional oversight of clinical care and facility relationships, you will also round 2-3 days a week at your own designated facility to care for patients who are experiencing difficulty participating in therapy due to pain or functional limitations.
Key responsibilities include:
Clinical point of contact for all Providers in your region
Clinical training to ensure all Providers display excellent clinical decision making and bedside manner
Time shadowing each Provider in the region
Regular clinical and facility meetings with facility staff
Focused assessments and developing individualized treatment plans in collaboration with facility primary care teams to optimize therapy participation and functional recovery.
Providing targeted interventions such as joint injections, Botox treatments, dry needling, and medication management based on patient-specific needs.
Supporting physiatrists on-site and operating with a high level of autonomy
Coordinating care and referring to other specialties or services for issues beyond the scope of physiatry
This role is best suited for a self-motivated and confident clinician who thrives in independent practice environments and is passionate about improving rehabilitation outcomes.
Qualifications:
5+ years as a Nurse Practitioner (NP) or Physician Assistant (PA)
Current, unrestricted license as a Nurse Practitioner (NP) or Physician Assistant (PA)
DEA licensed or willing to obtain DEA licensure
Skilled Nursing Facility experience
Physiatry, Pain Management or Physical Medicine & Rehabilitation experience
Comfort in performing ultrasound guided peripheral joint injections (knees, shoulders, hips)
A genuine passion for providing professional, personalized care in an autonomous environment
Relationship and value proposition building with key stakeholders
Strong communication skills, with the ability to collaborate with multidisciplinary teams
Experience in a clinical leadership or supervisory role
Willingness to travel regularly within the assigned region
