Please note that this position is for agency staffing support and home care services. The role will include: Assisting residents with daily activities such as bathing, dressing, and grooming. Providing mobility support and assisting with transfers. Helping with meal preparation and feeding, if needed. Monitoring and recording vital signs and health status. Providing emotional support and companionship to residents. Ensuring the safety and well-being of residents at all times. Assisting with medication reminders under supervision. Reporting any changes in residents' health or behaviour to healthcare professionals Act upon delegated tasks as required within your scope of practice This role will be direct at a long term care facility Qualifications Have one of the following qualifications: Valid Personal Support Worker Certificate from a Ministry of Training, Colleges, and Universities or the Ministry of Education approved program RN/RPN, Paramedic, Occupational Therapy and Physiotherapy student with a minimum successful completion of year one of the program Internationally licensed Nurse, Midwife or Physician with clearance to work as a PSW from the Ontario Personal Support Workers Association (OPSWA) Valid G2/G driver's license and access to a reliable car. The position requires daily travel of more than 1 hour, so 100% must have a vehicle. Candidates without a vehicle will not be considered. Physical ability to stand, bend, walk, crouch, kneel and reach, ability to lift 50lbs (23 kg) independently, ability to assist clients with standing, walking, and sitting; ability to reposition clients in bed Valid Standard First Aid and Level C CPR Updated immunization records (including completion of a two-step tuberculosis test within the last 6 months) Clear Vulnerable sector check is required upon hire Ability to interact with others in a caring, tactful and courteous manner Proficient verbal and written English communication skills Ability to use phone applications Requirements Vulnerable Sector Check completed within the last six months Submission of proof of two COVID vaccinations TB test completed within 6 months to a year Immunization record Up-to-date CPR and First Aid certification About Foundation Health Foundation Health is a patient-focused healthcare company specializing in family medicine, allied health, home care, self-testing products ranging from pregnancy and fertility to COVID-19 and flu, and virtual care.
