Job DescriptionJob Description Position Title: Nurse Practitioner (NP) Reports To: Clinical Manager Employment Type: Full-Time Position Overview: The Nurse Practitioner will provide holistic, culturally safe care to Indigenous clients and communities. The NP will work collaboratively with a multidisciplinary team to promote health and wellness, address chronic diseases, and support mental health initiatives in accordance with Indigenous health practices and principles. Key Responsibilities: Within the scope of practice as defined by the College of Nurses of Ontario (CNO), the nurse practitioner (NP) will provide comprehensive primary care services to clients of all ages; this includes primary prevention, health education, comprehensive physical exams, chronic disease management, antenatal care, and acute/episodic visits Visits take place predominantly in the clinic setting but may also include telephone contact and outreach visits as required Advocate on behalf of the client with regards to equitable access to “traditional healing” and “western” health services as directed by client Ensure effective and efficient use is made of all resources available in order to respond to the healing and wellness needs of the community Review client laboratory results, procedure reports and consultation notes and arrange appropriate follow-up/referrals as required Contact clients via telephone to discuss results as needed Documentation of all client contact, both direct and indirect Ensure that all client-based information is kept confidential at all times. Work with other health professionals to ensure that a broad range of services are available to clients Consultation with physician partner regarding client issues that are outside the NP scope of practice Collaborate with community partners to offer outreach services as required Qualifications: Ability to prioritize, manage time effectively and be very flexible in a dynamic work environment Excellent written and oral communication skills Current registration with the College of Nurses of Ontario. Bachelor of Science in Nursing required Competency in computer based programs (MS Office, EMR) Ability to work with others in an inter-professional team environment Asset Qualifications: Experience working in Indigenous health or communities is an asset Work Conditions: Full-time position with potential for flexible scheduling. Occasional weekend or evening shifts may be required. Physical demands include standing, walking, and lifting up to 50 pounds.
