The Chronic Disease Nurse Coordinator is an expert clinical nurse who will be working closely with the programs within Clinical Integration Unit 1 (this includes specialties of Diabetes, COPD and asthma, Intermediate Care Services, etc). In collaboration with Primary Health Care (PHC) colleagues, the Chronic Disease Nurse Educator is responsible for providing clinical supervision of staff, orientation, certification, continuing education, assisting in the development of clinical pathways, and participating in development of policies and procedures relating to general and specialty chronic conditions care. The incumbent will participate in program planning and evaluation. Primary responsibilities include ensuring services follow the Diabetes Canada Clinical Practice Guidelines, Canadian Thoracic Society Guidelines, etc. Experience Minimum 1 year supervisory experience five (5) years' recent nursing experience in Diabetes and Respiratory education Education Bachelor of Science in Nursing (BScN) Memberships College of Registered Nurses of Saskatchewan (CRNS) Other Education & Training RN special practice for insulin dose adjustment training Certified Diabetes Educator, is an asset Fully certified with insulin pumps is an asset. Certified Respiratory Educator, is an asset. Additional course in supervision and administration is an asset. Knowledge, Skills, and Abilities Uses independent judgment based on evidence-based practice. Knowledge in the principles of Population Health Promotion. Negotiation, conflict resolution and mediation skills. Demonstrates ability to accept and implement change. Ability to demonstrate appropriate level of professional conduct and judgement. Demonstrates flexibility by adjusting workload to accommodate variable needs Ability to be flexible in meeting diverse learning needs of adult learners Advanced knowledge of diabetes practices and principles. Proven skill in program or project planning and evaluation. Advance computer skills, including Microsoft Office (Excel, Word and Access). Knowledge in community health nursing theory and principles Uses critical analysis, reflective practice and assessment.
