The Thames Valley Family Health Team is one of the largest Family Health Teams (FHT) in Ontario. With more than 125 physicians across 21 sites within London/Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive, collaborative primary health care.
The Mental Health Counsellor provides assessment, ongoing treatment and management of biopsychosocial aspects of health and well-being for identified patients. The Mental Health Counsellor participates in the development, implementation, monitoring and evaluation of programs to meet the needs of the TVFHT patient population. Working in collaboration with an Inter-professional Healthcare Provider team, the Mental Health Counsellor assists health promotion, disease prevention, and chronic disease management across a patient’s lifespan. This role is currently providing virtual care.
Requirements:
Graduate of an accredited University
Registered and in good standing with a regulated health professional College (College of Nurses on Ontario or Ontario, Ontario College of Social Workers and Social Service Workers, College of Occupational Therapists of Ontario)
Minimum of five years clinical work experience
Extensive mental health clinical training, generally acquired through the completion of post graduate studies.
Knowledge and experience in mental health treatment and management (evidence-based treatments such as CBT)
Skills/Abilities:
An advanced level of mental health knowledge and experience working with adults, children, youth, & families, acquired through a minimum of 3-5 years supervised clinical practice in this area.
Understanding of Addictions care and navigation
Extensive mental health clinical training, generally acquired through the completion of post graduate studies (MSW, OT, RN, BScN, CPMHN(C) or equivalent) and membership in a professional college
Excellent clinical skills and a minimum of 3 years experience assessing and managing mental health presentations commonly seen in primary care.
Experience related to the delivery of efficient primary care, including the ability to share information and teach
Experience with developing and facilitating groups
Knowledge and proficiency in current, evidenced-based methods and practices of social work in primary care settings, with an emphasis on chronic disease management, health promotion and risk reduction
Ability to demonstrate an optimistic disposition toward new experiences and change in general
Monitor and evaluate service delivery for individual clients, communities, and populations by use of mental health & addictions measurement tools & technology
Knowledge of relevant health care legislation and community mental health resources
Understanding of social determinants of health and their impact on health and well-being
Strong leadership, organization, research, evaluation, time management and interpersonal skills
Demonstrated ability to work independently and collaboratively as a member of a multidisciplinary team
Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences
Active engagement in our performance management program
Great comfort level with providing in-person and virtual care
Experience in the use of an EMR, Microsoft Office and measurement based care platforms
Demonstrated ability to be open and non-judgmental
Effective interpersonal and communication skills (written/verbal)
Hourly Rate: (dependent on years of experience and profession)
Social Worker: $35.4072 - $43.1795
Occupational Therapist or Registered Nurse: $34.8138 - $42.4559
