Job Opportunity Mental Health Case Manager One Full-Time Contract to February 2028 (with the possibility of extension or becoming permanent) Status: Full-Time, Contract Starting Hourly Rate: $32.13 / Hour Office Hours: Monday - Friday - 8:30 am - 4:30 pm, 35 Hours per week Location: Aurora, Ontario L4G 3S9 Office Expectations; This position requires: A minimum of one in-office day per month to provide team coverage Attendance at additional in-office meetings, training sessions, supervision meetings, and all-staff events as required Applicants must reside within a reasonable daily commuting distance of the office and/or assigned service area and be able to attend the office as required for operational needs Posting Closes: Wednesday, September 2 at 4:00 pm EST Job Description In this role, you will provide comprehensive and collaborative community based case management response to individuals age sixteen and over with serious mental illness and/or addictions within York Region. Utilizing a Recovery Model framework, the Mental Health Case Manager will assist service users to identify needs and goals that will enhance the quality of their lives. Position Requirements Bachelor’s degree in social work or related discipline (i.e. Health Sciences) Experience; Minimum three years related experience Knowledge and practice of psychosocial rehabilitation approach and advanced clinical and assessment skills are assets Counselling, crisis prevention and intervention experience Experience working with individuals having serious mental illness is essential Excellent oral and written communication skills Second language is considered an asset An understanding of cultural diversity is considered an asset * The successful applicant will be required to provide a police clearance that meets agency standards * A valid G Drivers License, access to a reliable vehicle and adequate insurance coverage for business responsibilities, OR access to reliable transportation.
