Description Permanent part-time position (not locum). Augment your family practice with a long-term care (LTC) panel to provide challenging, diverse, and impactful work caring for the frail elderly. We are seeking an additional Long Term Care Practitioner to join the Southern Granville Park Lodge, a 107-bed LTC home currently supported by four LTCI practitioners. One practitioner is leaving, creating an immediate opening. Responsibilities Care for LTC residents as part of an interdisciplinary team (physicians, nurse practitioners, nurses, allied health, and care-home leadership). Provide holistic, resident-centered care including medication reviews and participation in 24/7 on-call duties. Attend care conferences and LTC-specific events and contribute to projects. Maintain chart documentation and participate in after-hours care program. Assist with care-home onboarding and mentorship of staff.
