The Role We're looking for a Licensed Prac tical Nurse (LPN) - Virtual to provide remote clinical support, telephonic triage, and care coordination. In this 100% remote role, you will interact with patients, families, and healthcare providers via phone, video, and electronic messaging to monitor treatment plans, coordinate care, and ensure clinical compliance. What You'll Do Conduct telephonic and virtual clinical intakes, health screenings, and wellness checks with patients and caregivers. Perform telephonic triage for symptom-based calls using established protocols and algorithms, directing patients to appropriate levels of care. Serve as a primary virtual point of contact for patients, families, and interdisciplinary care teams. Review and update individual plans of care under the supervision of Registered Nurses (RNs) or physicians. Track remote patient monitoring (RPM) data, vital sign trends, and clinical records to flag gaps in service or changes in status. Coordinate prescription refills, orders, and clinical authorizations with physicians, pharmacies, and insurance providers. Maintain accurate, timely clinical documentation in Electronic Health Record (EHR) platforms. Participate in virtual case conferences, team huddles, and firm quality/accreditation reviews What You Bring An active, unencumbered LPN license in your residing state is required (Compact Licensure preferred). Minimum of 1–2 years of clinical experience, with telehealth, telephonic triage, or remote care coordination experience preferred. Strong clinical assessment, telephone triage, and critical-thinking skills. Excellent verbal, written, and empathetic communication skills for telephone and video interactions. Proficiency with Google Workspace, EHR systems, and secure web-based communication tools. Dedicated home office setup with reliable, high-speed internet and high attention to HIPAA privacy standards.
