Position Summary
Reporting to the Manager, Comprehensive Intake and Care, the Intake Coordinator is responsible for the overall tasks involved with client intake conducting interRAI CHA pre-screener assessments, communicating effectively with internal and external resources, collecting statistical information, maintaining an accurate, relevant database. The position also provides backup support in the transportation program.
The Intake Coordinator is required to perform their duties in a manner consistent with Dixon Hall’s Mission, Vision and Values, ensure safety, and demonstrate customer service excellence.
The working hours are Monday to Friday: 9:00 AM to 4:00 PM.
Duties and Responsibilities
Program Delivery
Receive and review referrals & requests
Screen and assess clients’ eligibility for services and programs; provide consultation regarding appropriateness of services
Provide information to clients, caregivers, community members and service providers
Direct referrals to appropriate programs & services in the department or other community support agencies as necessary
Complete interRAI CHA pre-screener and basic intake assessments
Review and/or check the RMR system and Dove Care for referrals
Collaborate with program coordinators to initiate service
Coordinate with other partner organizations and internal resources
Department outreach, networking and program development
Administration
Establish and maintain statistical records which track referral sources and services/support to which clients are referred
Complete digital intake documents and create initial client file in database
Completed intake and client file is triaged and communicated to appropriate program staff
Provide information for funder reports as necessary
Organizational Responsibilities
Work collaboratively with other Dixon Hall staff and departments and volunteers
Operate in an effective, efficient and client focused manner
Actively participate in department and agency meetings and events
Work under the guidance of the agency and program policies and procedures, including the Mission, Vision and Values of Dixon Hall and the agency’s strategic direction.
Develop positive relationships with referring agencies, outreach engagements and inter-agency networking tables
Meet regularly with manager to discuss individual, team and program effectiveness
Undertake other tasks and responsibilities as assigned within the purview of the job description
Qualifications
Post-Secondary education in Social Services, Gerontology or a related field
A minimum of 3 years’ experience working with vulnerable or high-risk populations including seniors, adults with disabilities and/or with mental health issues, and individuals who are underhoused.
Excellent communication skills (written & verbal).
Strong assessment skills.
Knowledge of community-based programs and services
Ability to work independently and co-operatively both in consultation & jointly with others.
Strong organizational and planning skills.
Flexibility.
A second language is a definite asset.
Understanding and knowledge of the housing and health sector and services.
Ability to exercise good judgment, flexibility, creativity, and sensitivity to changing situations and needs.
Intermediate level of computer literacy utilizing MS Office applications.
Knowledge of AlayaCare will be considered an asset.
First Aid/CPR certification will be considered an asset.
Must be qualified to work in Canada legally for the employer.
Able to provide a Vulnerable Sector Screening check with a ‘Clear’ or satisfactory result that meets the operational requirements of the Agency.
