Pay Rate: $2200 Weekly Gross pay
based on a 40-hour work week (Includes Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement:
$0.54/mile for distance covered between patient home appointments

8 Hours; Days, 8a – 5p
Schedule:
40 hours / week guaranteed, 5 shifts per week, Mon-Fri
Position Requirements:
Registered Nurse: 2 years - Required
Experience with Hospice nursing in a home setting: 1 year within the last 3 years - Required
Home Care Home Base (HCHB) charting experience - Required
Must be able to do admissions - Required
Case Management experience - Highly preferred
Must have valid Driver’s License and Personal Auto Insurance
Must be Covid Vaccinated or provide medical or religious exemption
Education, License and Certification Requirements:
ASN or an equivalent – Required
BSN or an equivalent – Highly preferred
RN License for State applying or Compact if accepted - Required
BLS Certification from AHA or American Red Cross - Required
Job Description:
Provide and coordinate care for assigned hospice patients in the comfort of their homes.
Develop and implement nursing care plans, including Start of Care and admissions, as assigned.
Case management and provide clinical oversight to interdisciplinary teams
Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
Notify physicians of significant changes in status
Document and complete charting for all patient visits within 24 hours.
Provide Bereavement Support to patient families.
Territories to cover: Manchester & Salem (or) Wolfeboro & Ossipee
