Reporting to the Managing Director – Hospice, the RN Case Manager, provides skilled nursing care to hospice patients in their homes while coordinating individualized care plans with patients, families, and the interdisciplinary team (IDT). You'll serve as a trusted advocate, ensuring compassionate, high-quality care throughout the hospice journey.

What You’ll Do
Key Responsibilities
Perform comprehensive patient assessments and develop individualized care plans
Deliver skilled nursing care in patients homes in accordance with hospice standards
Coordinate care with patients, families, physicians, and the IDT
Supervise Hospice Aides and LVNs to ensure continuity of care
Maintain accurate, compliant clinical documentation
Complete patient admissions and participate in PRN on-call rotation
Manage patient schedules and prioritize changing care needs
Promote infection control, patient advocacy, and agency values
Assist with training and mentoring new clinical staff as needed
Commit to ongoing professional growth and clinical excellence through continuous education
What We Are Looking For
Qualifications
Graduate of an accredited Nursing program (Diploma, ADN, or BSN)
Active Texas RN license required; Current CPR certification
1–3 years of clinical nursing experience required; Hospice, ER, or OR experience preferred
Reliable transportation, valid Texas driver's license, and current auto insurance
Must pass background check and provide required TB and Hepatitis documentation
Additional Requirements
Ability to stand, walk, safely move patients, lift up to 50 lbs. and travel locally
Ability to work effectively in high-pressure, fast-paced environments
Strong clinical judgment and quick decision-making skills
Ability to meet the individualized psychosocial needs of patients and families
Excellent hand-eye coordination and manual dexterity
