Introduction
In this rewarding role, you will provide intensive, holistic support to families facing complex challenges, with the goal of strengthening parenting capacity, improving child safety, and keeping families safely together.
Key Responsibilities
Deliver intensive, therapeutic case management to families at high risk of children entering Out-of-Home Care (OOHC)
Engage and support families experiencing complex needs, including mental health, alcohol and other drug use, and domestic and family violence
Develop and implement strengths-based, family-led case plans, with ongoing monitoring and formal three-month reviews
Conduct weekly in-home visits to provide practical, consistent support
Build parental capacity through coaching, modelling and education to enhance communication, problem-solving and parenting skills
Provide hands-on support in the home, including establishing safe routines and household structure
Coordinate with services to deliver wraparound, collaborative care, including access to housing, healthcare, NDIS and Centrelink
Advocate for families’ rights, needs and wellbeing
Maintain accurate and timely case records in line with organisational and legislative requirements
Deliver group and individual training sessions for families
Provide crisis intervention and risk management when required
Support Aboriginal families in strengthening connections to kin, culture, community and Country, ensuring culturally safe practice
About You
To be successful in this role, you will bring:
Relevant qualifications in Social Work, Community Services, Psychology or a related field
Experience working with vulnerable families and children, particularly within intensive or preservation programs
Strong understanding of child protection, trauma-informed practice, and risk management
Demonstrated ability to engage families with complex needs in a strengths-based and culturally responsive way
Excellent communication, case management and organisational skills
Ability to work both independently and collaboratively within a multidisciplinary team
A current driver’s licence and willingness to conduct regular home visits
