Position Overview
We are currently looking for a committed and patient-focused
Family Physician
. The successful candidate will collaborate with a multidisciplinary team of Family Physicians and Nursing staff to deliver comprehensive primary care services to a diverse patient population. This role offers the opportunity to contribute meaningfully to community health while practicing in a supportive and modern clinical environment.

Key Responsibilities
Perform comprehensive patient assessments to diagnose and manage acute and chronic medical conditions
Develop and implement individualized treatment and care plans
Provide ongoing management of chronic diseases and coordinate referrals to specialists when appropriate
Deliver preventive care services, including health screenings, immunizations, and patient education
Maintain thorough and accurate medical documentation in compliance with regulatory and professional standards
Collaborate effectively with interdisciplinary healthcare professionals to ensure optimal patient outcomes
Qualifications & Skills
Experience practicing in hospital and/or community clinical settings
Strong understanding of primary care principles and best practices
Demonstrated compassion, professionalism, and strong interpersonal skills
Proficiency in medication management and clinical decision-making
Excellent communication and patient engagement abilities
Familiarity with medical terminology and electronic medical records (Accuro experience is an asset)
Ability to work collaboratively within a team-based care model
