Patient and Family Centred Care (PFCC) is the provision of care that is respectful of, and responsive to individual patient/family preferences, needs, and values and ensures that those values guide all decisions. Reporting to the Manager, Indigenous Collaboration, Equity & Inclusion the Indigenous Care Coordinator (ICC) is responsible for providing a range of health and mental health navigation, advocacy, discharge planning and supports services to Indigenous Peoples accessing health and mental health services at TBRHSC. The ICC will work as a liaison between the Hospital and local community organizations in order to meet the needs of Indigenous patients. The ICC works as part of an interprofessional team, including other ICCs who report to Anishnawbe Mushkiki, Grand Council Treaty #3, and Matawa Health Co‑Operative. The goal of the program is to improve equitable access to care for Indigenous patients and support their transitions in care through rigorous discharge planning processes and supports that leverage community/region‑based services that are culturally appropriate and safer. The ICC will work with local communities to support the smooth transition of patients as appropriate. The ICC will be a generalist of services available for Indigenous Peoples throughout Northwestern Ontario. The successful candidate is someone who is passionate about improving the health and mental health outcomes of Indigenous Peoples by providing culturally appropriate and safer care EMPLOYMENT REQUIREMENTSEducation/Experience University degree or college diploma in a health or social sciences, or related field Demonstrated experience working with Indigenous Peoples and communities while providing culturally safe, trauma informed, and patient‑centered care (community references required) Knowledge of Indigenous cultures, traditions, histories, and protocols gained through lived experience Three (3) to five (5) years of work experience in the Indigenous health, advocacy, community outreach or a related field Current unrestricted registration in good standing as a Registered Nurse or Registered Practical Nurse with the College of Nurses of Ontario preferred Skills/Abilities Demonstrates the ability to effectively manage multiple competing and frequently shifting priorities simultaneously, maintaining organization, focus, and attention to detail while adapting to changing demands and timelines Demonstrated ability to work in a collaborative team approach with patients, management, medical and support staff Demonstrated awareness and adherence to established standards of professional practice, personal conduct and safety Experience or knowledge of the health care system, local and regional mental health and addictions resources and availability and community resources, services and supports Excellent interpersonal, oral and written communication skills Experience navigating Non‑Insured Health Benefits and Jordan’s Principle Experience in case management, discharge planning and understanding in home care and/or community‑based service delivery Experience in crisis intervention and de‑escalation techniques Analytical problem solving and computer skills Assets Fluent in Ojibwe and/or Oji‑Cree Certified in Standard First Aid and CPR Current certification in BCLS Salary Range: $43.840 to 54.800 The Health Sciences Centre strives to ensure the safety and security of the patients, visitors, employees and assets financial and otherwise.
