They monitor and coordinate treatment plans with patients to educate them about their illness and connect them with various medical professionals as needed. A Care Coordinator, also known as a Patient Care Coordinator, plays a crucial role in managing patients care. Here are the key aspects of their job: Scheduling and Organizing: A care coordinator is responsible for assisting with scheduling, organizing, and managing various aspects of a patients healthcare maintenance and treatment. They facilitate communication between staff, patients, family members and members of the multidisciplinary team (MDT). Patient Advocacy and Education: Care Coordinators help patients complete onboarding paperwork and understand their treatment plans. They educate patients about available resources and options for managing their health. Collaboration and Coordination: They work closely with multidisciplinary healthcare teams, (MDTs), to ensure seamless coordination and delivery of care. Coordinating between client, families, specialists and other healthcare providers is essential for high-quality patient care. Monitoring Progress: Care Coordinators develop patient goals and monitor progress toward achieving them. They maintain patient confidentiality and ensure quality care. Job Description: Position: Patient Care Coordinator (RN/RPN/LPN) Location: Edmonton South Responsibilities: Coordinate operational activities of assigned patient care duties. Facilitate cooperative and collaborative relationships between nursing management and staff when in facility, or with family members in community. Maintain 24-hour accountability for compliance with nursing care standards. Work full-time (primarily Mon-Fri, with on-call availability 24 hours a day). Requirements: Active RN license (BSN preferred). Minimum 3-5 years of experience in acute med-surg nursing, LTC or home care experience is also considered an asset. Management experience preferred. BLS required & ACLS certification recommended.
