About the role
Join the Transitional Aged Care Service team at Calvary Kogarah. We are seeking a dedicated Social Worker (Level 1–2) to join our supportive team in a temporary part-time role, working 64 hours per fortnight until January 2027. This is a great opportunity to make a meaningful difference in the lives of older people as they transition from hospital back to the community.
Your main duties will include:
Plan, coordinate and deliver high quality social work care to patients/clients of Calvary Health Care Kogarah Transitional Aged Care Service (TACS).
Work with the Multidisciplinary team to assist clients to achieve optimal outcomes
