Housing First Case Manager
to promote independence and quality of life for individuals or families experiencing chronic homelessness through the coordination of appropriate services; to provide consistent and ongoing community-based support as needed by the person supported using an intensive case management approach; to provide responsive supports and services to the person's multiple and changing needs within the context of a trusting and productive relationship that assists the person to achieve goals, and stabilize his/her life; to embrace a collaborative approach to service delivery that provides hands-on, flexible and low barrier support that assists the person to maintain housing.
