Position: Circle of Care Coordinator
Department: Health & Community Wellness
Employment Status: Contract (ending March 31/27)
Salary: $31.34- $37.46/HR 35 Hours per Week
Location: Naughton, ON

POSITION SCOPE:
The Circle of Care Coordinator will be responsible for the case management and direction of individualized treatment planning, service delivery, and treatment review for each client. The Circle of Care Coordinator will take the Lead in the review of clinical issues and program policies and procedures, as it relates to the Circle of Care.
REQUIRED QUALIFICATIONS:
Bachelor of Arts Degree or Diploma in a Social Work or Equivalent
3-5 years work experience in social/health setting
KEY RESPONSIBILITIES:
Is the first point of contact for client referrals and coordinate childrens clinical therapeutic services,
Will provide assistance to individual(s)/families through the case management model,
Supports evaluation of program services to enhance patient outcomes and acts as a positive change agent to implement clinical initiatives to increase quality client care
Develop and implement programs that address the needs identified in the Community Health Plan
