In accordance with established vision and values of the organization, the Patient Care Coordinator (PCC) practices in accordance with the standards of professional practice and code of ethics as outlined by the BC College of Nurses and Midwives (BCCNM) as well as within a patient and family centered care model.
The PCC is responsible for the coordination of day to day operations and quality patient care within a designated clinical area(s). This position supports the seamless movement of patients through the healthcare system and in accessing appropriate resources. The PCC supervises as well as serves as a role model and a resource to staff for clinical decision-making, clinical assessments of patients and in the counselling and education of families. Reviews, analyses and makes recommendations on the development of administrative systems such as policies, procedures and standards of patient care, adequate staffing levels, and a continuous quality improve
